This case describes a 52-year-old woman admitted to the emergency room in a comatose state. An arterial gas sample revealed an extreme metabolic acidosis (pH 6.36). It was presumed that the acidosis originated from ingestion of ethylene glycol in engine coolant. This patient recovered fully following dialysis, infusion of ethanol, inotropics and respiratory support. It is emphasised that such intoxication must be considered in unexplained cases of metabolic acidosis.
